The vacancy
Could you be our new Peer Navigator? We are looking for someone with lived experience of homelessness to join our team as Peer Navigator. The new role will see you working in collaboration with Leeds Community Health Inclusion Team (HIIT) and Leeds Palliative Care Inclusion Team (LPCIT).
You will be responsible for building therapeutic relationships with patients and acting as a link between them and healthcare professionals. Responsibilities include:
• Befriending/ mentoring
• Accompany patients to their appointments
• Supporting patients to navigate through the various health systems • continued support whilst at home, hospital, hospice etc
• Supporting HIIT and LPCIT in the development of services across Leeds
We are looking for someone who is kind, friendly, listens well, can empathise and understand the views of others, has a good sense of social justice and can work well within teams. The successful applicant will receive external supervision and support by an expert in mentoring people with lived experience.
